fastmail settings?

TRYING TO SET UP T MOBILE FASTMAIL ACCOUNT SETTINGS, cAN SOMEBODY PLEASE TELL ME WHAT TO PUT IN THE FOLLOWING FIELDS:
1) SERVER INFO
2) SERVER TYPE

login: your email (with @xxx.xxx)
password:

IMAP:imap.proxy.fastmail.fm
SMTP:smtp.proxy.fastmail.fm
(I am a paid user, so I can use proxy and set the port to anything I want. COmpany has port 25 blocked, so I am using 80 now. You also need to be a paid used to use its SMTP)

Hope this helps. I am using these setting in Outlook 2003 on my work PC.
