I actually had the same problem about a month ago. I could not use the phone unless I was on speaker or used a handsfree set. I called Customer Care at At&T, and they said I could replace my phone. I do not have any warranty on the phone, but had recently bought it at the end of May. So, I think there was a 90 warranty that I could still use. I just had it replaced.

Eddie's solutions worked great for me. Put the headphones in, take the battery out and then put the battery back in with the headphones still plugged in, once it boots up just take the headphones out and your golden. Eddie your a life saver (well more like a money and time saver), really appreciate the great post.
