If someone gets a blank copy of the rebate form, could you please make it available to those who did not get one in their box? I would appreciate it immensely! Thanks.

This is all I could find on their site after some searching:

You can also call 611 Customer service, and they can email or snail-mail it to you.

I got mine from the store today and still didnt get a rebate form

I looove how they do the rebate thing, it took me 3 1/2 months or better to get mine back from the 9k. Ridiculous. Anyway, hope you get your form.

In searching the net for this form, I found it on a partner BB forum site... not sure if I am allowed to say, but if you want the form, PM me and I'll tell you the site.

I can promise its completely a legit ATT rebate form.
